Answers to Your Most Common Questions

What happens during the consultation?

Your consultation includes a full exam, diagnostic imaging (like X-rays or a 3D scan if needed), and a review of your medical history. Dr. Jay or Dr. Michael Malmquist will explain your diagnosis, discuss treatment options, and answer any questions you have.

Do I need a referral to schedule a consultation?

While a referral from your general dentist is helpful—especially if they’ve already taken X-rays—it is not required. You can contact us directly to schedule your visit.

Is there a fee for the consultation?

Yes, there is a consultation fee. However, for implant, TMJ, or orthodontic consultations, the fee will be credited toward your treatment plan if you decide to move forward.

What should I bring to my appointment?

Please bring any relevant X-rays, your completed referral form (if applicable), a list of your current medications/allergies, and your dental or medical insurance information—including forms and benefit details.

Will I receive a treatment plan at the first visit?

In most cases, yes. If your needs are complex and require additional records (like 3D imaging or dental models), your treatment plan will be finalized once those are reviewed. Either way, you’ll leave with a clear understanding of your next steps.

Will you coordinate care with my dentist or specialist?

Yes! If your treatment involves another dental or medical provider, our Treatment Coordinator will handle all communication and scheduling to ensure seamless care.
